Use this Form to Submit Your Health Education Material

Instructions

We suggest you print out a blank form to use as a working copy. The information you provide will be entered into the H.E.R.E. in Washington on-line clearinghouse. The Materials Exchange is a way for community health professionals in Washington to share educational materials they have developed. The materials you submit must be produced by your agency or adapted for the specific needs of your community. Please fill out all parts of the form. Fill out a separate form for each item you wish to submit. Feel free to make copies of this form and share it with others in your agency. Please mail one copy of each educational material to:
